IMPORTANT ANNOUNCEMENT – CLINICAL SYSTEM UPGRADE
We are upgrading our electronic Clinical System shortly.
To facilitate this we are now not registering patients as ‘Permanent’ patients from now until 9th September 2019.
Instead, until 23rd August 2019, we will register patients as ‘Temporary’ Patients.
You will be switched to ‘Permanent’ patient status after 9th September 2019.
If you need an appointment in this time please contact your previous GP to email a medical summary to firstname.lastname@example.org or provide one with your registration forms.
Between 26th August 2019 and 9th September 2019 we will accept registration forms but we will not be able to register you until after 9th September 2019.
This service will be suspended from Friday 16th August 2019. We will accept online services registration forms but are now not registering patients for Online Services until end of September 2019

New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.
Please fully complete the New Patient Pack, and return to the surgery along with a Photo ID and 2 proofs of address (for persons over the age of 18).
Proof of address can be utility bills, bank/credit card statements or rental agreement/electoral roll confirmation.
For children under 6 years of age we also need to borrow the red book for a short period of time.
If you are a Carer; please also complete this form to identify yourself as such. With your permission we will pass your details onto the local Carers Support Organisation who may be able to offer you extra support.
Registrations usually take a week to be processed and once completed all new patients are invited to attend a health check with a member of the healthcare team.
Medical treatment is available from the date of registration. Please contact reception for further information.
